You may bring it with you to your free family law consultation or email it info@HoustonDivorceSource.com prior to your arrival. It is important that you are thorough in your responses as it helps us better evaluate your case.

Financial Information Statement (active clients only – all cases)

Please download and print out the Financial Information Statement and fill it out completely. You may submit it to our office in person or email it info@HoustonDivorceSource.com. Once complete, you must also submit your (2) most current pay stubs along with the past two years of tax returns. Remember to submit all documents to us on or before the date assigned by our office so that we may properly prepare them for submission to the court in accordance with local rules.

Inventory and Appraisement (active clients only – Divorce)

Please download and print out the Inventory and Appraisment and fill it out completely. You may submit it to our office in person or email it info@HoustonDivorceSource.com. For each item of real property identified (house, lot, ranch, etc.) please submit all records, deeds and contracts along with a current mortgage statement if applicable. For each bank account, credit card or other debt listed, please submit the most current statement. It is important to list all assets whether acquired prior to or during the marriage. Remember to submit all documents to us on or before the date assigned by our office so that we may property prepare the them for submission to the court in accordance with local rules.
